Conditions of Participants:
All participants in this program, conducted by South Shore Field Hockey Club at the host site, must agree to abide by this Liability Waiver and Code of Conduct. By electronically selecting “Agree”, you acknowledge that you have read it, understand it, and are willing to abide by the Liability Waiver, Code of Conduct, and the additional South Shore Field Hockey Club Personal Health & Safety Protocol.
Liability Waiver:
I acknowledge that all South Shore Field Hockey events are privately run and have no affiliation or partnerships with the venues and facilities at which they are operated.
I agree to hold the South Shore Field Hockey Club location, venue and/or host, its facilities, management and employers and its representatives (officers, agents, employees, coaches, counselors and officials) harmless from and against any and all claims for injury, costs, liability, damages or loss to person and property which may be sustained or occur at and on the premises of the event, whether or not they are due to negligence.
Participation:
As parent/guardian, I give consent for my child to participate in all sports activities with South Shore Field Hockey Club. I hereby acknowledge there are risks of injury involved in my child's sports participation. My child is physically able to participate in strenuous activities and attend this event.
Medical Treatment:
I hereby authorize South Shore Field Hockey Club, and its representatives (officers, agents, employees, coaches, counselors and officials) to seek emergency medical treatment on behalf of my child in the event that, in the opinion of South Shore Field Hockey Club, my child is in need of such treatment.
South Shore Field Hockey Club, has the following medical treatment procedure in place for all of its events including Competitions, caring for a participant (your child) when they become sick or injured while participating in the events:
The event director / first responder / certified athletic trainer will attend to the participant.
Upon recommendation of the event director / first responder / certified athletic trainer, further medical treatment may be attained.
In pursuing further medical treatment, South Shore Field Hockey Club will request the club team coach to contact the parent(s) / guardian(s) of the sick or injured player.
If there is no contact with parent(s)/guardian(s), based upon the medical judgment of the event director / first responder / certified athletic trainer, parent(s) / guardian(s), and/or player’s physician, the child may be admitted to a local medical facility. If necessary, the player will be transported to the medical facility via ambulance.
I consent to any treatment and/or diagnostic procedures which may be carried out based on the medical judgment of the first responder / certified athletic trainer. I also agree that I will indemnify and hold harmless South Shore Filed Hockey Club and further release South Shore Field Hockey Club and its representatives for any damages sustained by my child in connection with providing medical treatment.
I understand that South Shore Field Hockey Club does NOT provide medical insurance. I further agree that I will be responsible for the payment of any and all medical treatment, associated transportation costs and/or medicines of any nature which may arise in connection with any sickness or accident which may occur during the registered event, whether such expense is incurred during or subsequent to the tournament.
Assumption of Risk and Waiver of Liability Relating to Communicable Diseases including COVID-19
The novel coronavirus, COVID-19, is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, Massachusetts’s Governor has issued various emergency declarations limiting activities, closing schools, and requiring social distancing. The emergency declarations prohibited the congregation of large groups of people. South Shore Field Hockey Club, and its host location, will follow the Governor’s guidelines and will take reasonable steps to enforce them at all on campus events. However, South Shore Field Hockey Club cannot guarantee that anyone attending the event will not become infected with COVID-19.
By my/my child’s attendance at any training, events, and activities ("Activities") associated with South Shore Field Hockey Club, I acknowledge and agree that:
● I and my child(ren) voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, related to real or potential COVID-19 exposure or infection, that I or my child(ren) or anyone attending with you may experience or incur in connection with attendance at the event.
● I acknowledge Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist.
● I acknowledge the contagious nature of COVID-19.
● I knowingly and voluntarily assume all such risks of being exposed or infected with COVID-19 or other communicable diseases, both known and unknown, even if arising from the negligence of the releases or others, and assume full responsibility for my child(ren)'s participation.
● I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases.
● If, however, I observe any unusual or significant hazard during my presence or participation, I will remove myself & my child(ren) from participation and bring such to the attention of the nearest South Shore Field Hockey Club staff-member immediately.
I release, waive, and discharge any and all claims that I or my heirs, assigns, personal representatives, and next of kin may have now or in the future against South Shore Field Hockey Club and its officers, directors, employees, contractors, subcontractors, sponsors, suppliers, national organizations or associations, representatives, agents, affiliates, insurers, successors and assigns, other participants, teams, sponsors, and advertisers of South Shore Field Hockey Club Activities; and, if applicable, owners and lessors of the premises used by South Shore Field Hockey Club (collectively the "Releases") for any liability, expenses, loss or damage to person or property, injury, death or disability suffered from or in connection with any and all illness, disability, death, or loss or damage to person or property, whether arising from the negligence of releases or otherwise, to the fullest extent permitted by law.
On behalf of myself, any person for whom I have legal decision-making authority or rights attending with you, and anyone else attending with you, you also release, covenant not to sue, discharge, and hold harmless, Weymouth Youth Field Hockey Inc., its officers, directors, employees, contractors, agents, partners, and representatives of and from the Claims.
2 (South Shore Field Hockey Club): Player Liability Waiver & Code of Conduct
My child/I will NOT attend any events, competitions, sessions, training, etc. IF I have any reason to feel as if I or my child(ren) may have COVID-19 or any of the above or other communicable diseases. I will maintain safe social distancing, sanitize my own equipment, etc. and follow all guidelines for the health and safety of staff and other members.
Code of Conduct:
Those participants perpetuating damage to the facility will pay for any physical damage to the facility.
There will be no possession or use of alcoholic beverages at the host site by anyone under the legal drinking age; or anyone if prohibited by the host site.
There will be no possession or use of any prescription drug or USOC banned substance unless the participant is currently under physician’s care and the medication is required for treatment of an illness or injury (documentation is required).
Unacceptable behavior will not be tolerated, including but not limited to:
Any act considered to be an offense under federal, state, local and USOC laws and rules.
Vandalism, damaging of property, unauthorized use of equipment or property, or theft.
Unsportsmanlike conduct.
Inappropriate horseplay or other misbehavior which physically endangers any person or property.
Persons present while any violation of the Code of Conduct occurs must leave the area immediately or be considered a participant by choice.
Any person observing or having knowledge of an activity, which may result in serious bodily harm to a participant, must immediately report to a South Shore Field Hockey Club staff-member.
Consequences:
Violation of the Code of Conduct may result in:
Participant(s) responsible for property loss being liable to pay all replacement/repair costs.
Partial or full restriction of movement at the host site.
Expulsion from the event with participants being sent home at his/her own expense.
Team-Expulsion from the event.
Process:
All violations shall be reported as soon as possible to the South Shore Field Hockey Club site director.
Upon notification of a violation, the director and appropriate staff members shall decide upon the mode of disciplinary action.
The South Shore Field Hockey Club site director and/or South Shore Field Hockey Club program director shall have authority to enforce any of the above listed consequences at the time of the violation.
Photography Release:
I grant South Shore Field Hockey Club the right to take photographs and film of my child in all South Shore Field Hockey Club programs/activities. I authorize South Shore Field Hockey Club, its assigns, and transferees to copyright, use, and publish the same photographs in print and/or electronically. I agree that South Shore Field Hockey Club may use such photographs of my child, with or without my child’s name, and for any lawful purpose, including, but not limited to, publicity and Web content.
